How to copyright our names

Hi!
A couple of questions re copyright of names, as we’re told it’s important to do, but I can’t find instructions –
1. How is this done (is there a template/example)?
2. Do I need to copyright both my maiden name and my married name?
3. Internet searches say that there’s no formal registration required – what do we do with them?Thanks 🙂
In the Affidavit of Status and Life, look for Exhibit ‘E’. It is a Copyright Notice.
Yes, maiden and married names can be copyrighted, as per the notice.
You’ll find a copy of the affidavit under the Resources tab.
